By submitting this form, the participant or their guardian acknowledges the inherent risks involved in Silat training and events, including potential injuries. In return for the opportunity to participate, they agree to absolve the Silat Institute, its staff, Abdul-Malik Ahmad, and related parties of any responsibility for injuries incurred during classes, events, or in transit. This waiver applies to all related claims. Participants under 18 must have this form signed by a parent or guardian. The participant is required to have completed a comprehensive physical exam by a licensed physician in the last year, ensuring they are physically and mentally prepared for Silat activities. Should any legal liability fall upon the Silat Institute or its affiliates, any resultant compensation will not exceed the lesser of actual medical costs or $2,000. Note that all payments are final and non-refundable. Furthermore, the participant, along with their parent or guardian, consents to the use of any photos, videos, interviews, and other media taken during events for promotional activities by the Silat Institute across various platforms, including Facebook, Instagram, and YouTube, without any entitlement to compensation.
